Je to typ nervovej bolesti a predpoklad\u00e1 sa, \u017ee takmer 7 % celkovej popul\u00e1cie trp\u00ed r\u00f4znymi formami probl\u00e9mov s neuropatickou boles\u0165ou. Pr\u00edznaky neuropatickej bolesti sa l\u00ed\u0161ia od jednej osoby k druhej \u2013 v z\u00e1vislosti od po\u0161kodenia nervovej \u0161trukt\u00fary. CRPS II (komplexn\u00fd region\u00e1lny bolestiv\u00fd syndr\u00f3m) je typ neuropatickej bolesti, ktor\u00e1 za\u010d\u00edna v d\u00f4sledku po\u0161kodenia nervov. V tak\u00fdchto pr\u00edpadoch sa boles\u0165 \u0161\u00edri po obrovskej ploche tela mimo miesta po\u0161kodenia nervov.<\/p>\n\n\n\n<p>Lie\u010dba<br>V\u00e4\u010d\u0161ina konven\u010dn\u00fdch liekov ur\u010den\u00fdch na lie\u010dbu neuropatick\u00fdch bolest\u00ed nie je tak\u00e1 \u00fa\u010dinn\u00e1. Maj\u00fa tie\u017e ne\u017eiaduce reakcie, n\u00edzku \u00farove\u0148 tolerancie a m\u00f4\u017eu ovplyvni\u0165 kvalitu \u017eivota. Centr\u00e1lna neuropatick\u00e1 boles\u0165 vznik\u00e1 v d\u00f4sledku ak\u00e9hoko\u013evek druhu poruchy alebo po\u0161kodenia miechy alebo mozgu. A perif\u00e9rna neuropatick\u00e1 boles\u0165 je sp\u00f4soben\u00e1 v d\u00f4sledku poruchy alebo poranenia, ktor\u00e9mu \u010del\u00ed ktor\u00e1ko\u013evek in\u00e1 \u010das\u0165 zmyslov\u00e9ho nervov\u00e9ho syst\u00e9mu.<\/p>\n\n\n\n<p>Pod\u013ea r\u00f4znych epidemiologick\u00fdch \u0161t\u00fadi\u00ed sa dospelo k z\u00e1veru, \u017ee subjekty s neuropatickou boles\u0165ou nedost\u00e1vaj\u00fa spr\u00e1vnu lie\u010dbu. Hlavn\u00fdm d\u00f4vodom je, \u017ee vykonan\u00e1 diagnostika je nedostato\u010dn\u00e1, a preto je predp\u00edsan\u00e1 lie\u010dba ne\u00fa\u010dinn\u00e1. Neuropatick\u00e1 boles\u0165 vedie k r\u00f4znym komplik\u00e1ci\u00e1m, ak sa nelie\u010di v\u010das. Je potrebn\u00e9 vyvin\u00fa\u0165 lep\u0161ie diagnostick\u00e9 postupy a potom predpisova\u0165 pacientom s neuropatickou boles\u0165ou spr\u00e1vnu lie\u010dbu. Neuropatick\u00e1 boles\u0165 m\u00f4\u017ee \u010falej vies\u0165 k zhor\u0161eniu fyzick\u00fdch funkci\u00ed, poruch\u00e1m sp\u00e1nku a depresii. Pre ostatn\u00fdch \u010dlenov rodiny je tie\u017e ve\u013emi \u0165a\u017ek\u00e9 zvl\u00e1dnu\u0165 pacienta kv\u00f4li finan\u010dn\u00fdm a zdravotn\u00fdm probl\u00e9mom.<\/p>\n\n\n\n<p><strong>Farmakopunkt\u00fara v\u010delieho jedu: \u00da\u010dinn\u00e1 lie\u010dba syndr\u00f3mu komplexnej region\u00e1lnej bolesti<br><\/strong>Na lie\u010dbu CRPS (syndr\u00f3m komplexnej region\u00e1lnej bolesti) nie je k dispoz\u00edcii \u017eiadna spr\u00e1vna terapia. V tejto klinickej \u0161t\u00fadii bol vybran\u00fd pacient so syndr\u00f3mom komplexnej region\u00e1lnej bolesti vo veku 29 rokov, u ktor\u00e9ho sa sympt\u00f3my vyvinuli po chirurgickom o\u0161etren\u00ed 4. a 5. prsta. Pacient dostal r\u00f4znu lie\u010dbu bolesti, ale jeho sympt\u00f3my nebolo mo\u017en\u00e9 kontrolova\u0165. Po n\u00e1v\u0161teve nemocnice Dunsan k\u00f3rejskej medic\u00edny na univerzite Daejeon bol jeho pr\u00edpad vyhodnoten\u00fd na d\u00f4kladn\u00e9 vy\u0161etrenie. Znamenalo to, \u017ee pacient mal siln\u00fa precitlivenos\u0165 na dotyk \u2013 be\u017en\u00fd pr\u00edznak neuropatickej bolesti. Okrem t\u00fdchto probl\u00e9mov sa pacient s\u0165a\u017eoval aj na depresiu, vyr\u00e1\u017eku a dyspepsiu.<\/p>\n\n\n\n<p>Nesk\u00f4r, ke\u010f bola lie\u010dba NRS a BVP ukon\u010den\u00e1, numerick\u00e1 hodnotiaca stupnica nezaznamenala \u017eiadnu boles\u0165 v prstoch na noh\u00e1ch, preto\u017ee mala hodnotu 0. To znamenalo, \u017ee u\u017e nebolo potrebn\u00e9 u\u017e\u00edva\u0165 lieky proti bolesti.<\/p>\n\n\n\n<p>Odpor\u00fa\u010da sa sledovanie po 4 mesiacoch. Zistilo sa, \u017ee ne\u010delil \u017eiadnym sympt\u00f3mom bolesti ani ne\u017eiaducim \u00fa\u010dinkom farmakopunkt\u00fary v\u010delieho jedu.<\/p>\n\n\n\n<p>Pod\u013ea v\u00fdsledkov tohto experimentu mo\u017eno poveda\u0165, \u017ee BVP je u\u017eito\u010dn\u00e1 terapia na lie\u010dbu subjektov s CRPS. V\u010del\u00ed jed je jednou z najbezpe\u010dnej\u0161\u00edch mo\u017enost\u00ed na lie\u010dbu r\u00f4znych z\u00e1palov\u00fdch ochoren\u00ed a u\u017e dlho sa pou\u017e\u00edva aj s in\u00fdmi liekmi. Je zauj\u00edmav\u00e9 poznamena\u0165, \u017ee v\u010del\u00ed jed nie je nov\u00fd vyn\u00e1lez. Pou\u017e\u00edvali ho aj starovek\u00ed \u013eudia na lie\u010dbu r\u00f4znych chor\u00f4b.<\/p>\n\n\n\n<p>V\u010del\u00ed jed sa teraz pou\u017e\u00edva na lie\u010dbu chronick\u00fdch chor\u00f4b, ako s\u00fa neurologick\u00e9 poruchy. Sk\u00famanie sa uskuto\u010dnilo prostredn\u00edctvom farbenia 4,6-diamidino-2-fenylindolom, testu 3-(4,5-dimetyltiazol-2-yl)-2,5-difenyltetraz\u00f3liumbromidu, testu uvo\u013e\u0148ovania lakt\u00e1tdehydrogen\u00e1zy, aktivity kasp\u00e1zy-3 , reverzn\u00e1 transkripcia-polymer\u00e1zov\u00e1 re\u0165azov\u00e1 reakcia a Western bloty.<\/p>\n\n\n\n<p>Bolo pozorovan\u00e9, \u017ee bunky o\u0161etren\u00e9 H202 zn\u00ed\u017eili apoptotick\u00e9 vlastnosti a \u017eivotaschopnos\u0165 buniek a zv\u00fd\u0161ili tvorbu kasp\u00e1zy-3. Lie\u010dba melit\u00ednom potla\u010dila fragment\u00e1ciu apoptickej DNA a posilnila \u017eivotaschopnos\u0165 buniek. Melitt\u00edn tie\u017e oslabil H202, presved\u010dil pokles produkcie prote\u00ednu a mRNA antiapoptotick\u00e9ho faktora Bcl-2. Okrem toho sa pozorovalo, \u017ee melit\u00edn zabr\u00e1nil tak expresii prote\u00ednu, ako aj mRNA indukovanej H202 z Bax viazanej kasp\u00e1zy-3 a proapoptotick\u00fdch faktorov.<\/p>\n\n\n\n<p>T\u00e1to klinick\u00e1 \u0161t\u00fadia nazna\u010duje, \u017ee melit\u00edn m\u00e1 mimoriadne \u00fa\u010dinn\u00e9 terapeutick\u00e9 \u00fa\u010dinky a mo\u017eno ho pou\u017ei\u0165 na prevenciu a kontrolu sympt\u00f3mov neurodegenerat\u00edvnych ochoren\u00ed.<\/p>\n\n\n\n<p><strong>Aktiv\u00e1cia miechov\u00fdch \u03b12-adrenoreceptorov pomocou stimul\u00e1cie zrieden\u00fdm v\u010del\u00edm jedom zni\u017euje chladov\u00fa alod\u00fdniu pri neuropatickej bolesti\u2026<br><\/strong>Ako u\u017e bolo spomenut\u00e9, neuropatick\u00e1 boles\u0165 m\u00e1 r\u00f4zne prejavy \u2013 jedn\u00fdm z nich je studen\u00e1 alod\u00fdnia. Je to charakteristick\u00e1 \u010drta tejto chronickej bolesti a s\u00fa\u010dasn\u00e1 klinick\u00e1 \u0161t\u00fadia sa to\u010d\u00ed okolo nej. Pomocou tejto \u0161t\u00fadie sa zistilo, \u010di jednorazov\u00e9 alebo opakovan\u00e9 o\u0161etrenie DBV (rieden\u00fdm v\u010del\u00edm jedom) m\u00f4\u017ee pom\u00f4c\u0165 zn\u00ed\u017ei\u0165 chladov\u00fa alod\u00fdniu u potkanov CCI (chronick\u00e9 z\u00fa\u017eenie) sedacieho nervu. A tie\u017e sa analyzovalo, \u010di bol tento \u00fa\u010dinok sprostredkovan\u00fd miechov\u00fdmi adrenergn\u00fdmi receptormi alebo nie.<\/p>\n\n\n\n<p>Jedna injekcia zrieden\u00e9ho v\u010delieho jedu 2,5 mg\/kg bola podan\u00e1 do Zusanli acupoint takmer 2 t\u00fd\u017edne po CCI. Opakovan\u00e1 lie\u010dba zrieden\u00fdm v\u010del\u00edm jedom v d\u00e1vke 0,25 mg\/kg sa pod\u00e1vala po\u010das 14 dn\u00ed hne\u010f po 15 d\u0148och lie\u010dby CCI. Zistilo sa, \u017ee jedna d\u00e1vka zrieden\u00e9ho v\u010delieho jedu v n\u00edzkej d\u00e1vke 0,25 mg\/kg nevyvolala alodynick\u00fd \u00fa\u010dinok proti chladu. Ale na druhej strane vysok\u00e1 d\u00e1vka zrieden\u00e9ho v\u010delieho jedu 2,5 mg\/kg p\u00f4sobivo zn\u00ed\u017eila chladov\u00fa alod\u00fdniu.<\/p>\n\n\n\n<p>Okrem toho v d\u00f4sledku tejto vysokej d\u00e1vky zrieden\u00e9ho v\u010delieho jedu bol jeho \u00fa\u010dinok nakoniec blokovan\u00fd intratek\u00e1lnou predbe\u017enou lie\u010dbou antagonistom \u03b12-adrenoceptora (idazox\u00e1n), ale nie antagonistom \u03b11-adrenoceptora (prazosin) alebo neselekt\u00edvnym antagonistom \u03b2-adrenoceptora (propranolol).<\/p>\n\n\n\n<p>Okrem toho spolo\u010dn\u00e9 pod\u00e1vanie n\u00edzkej d\u00e1vky zrieden\u00e9ho v\u010delieho jedu 0,25 mg\/kg a agonistu a2-adrenoceptora (klonid\u00ednu) pomohlo zn\u00ed\u017ei\u0165 chladov\u00fa alod\u00fdniu. Zatia\u013e \u010do opakovan\u00e9 o\u0161etrenie vykon\u00e1van\u00e9 n\u00edzkou d\u00e1vkou zrieden\u00e9ho v\u010delieho jedu nevykazovalo \u017eiadny motorick\u00fd deficit, ktor\u00fd by v\u00fdznamne inhiboval chladov\u00fa alod\u00fdniu za\u010d\u00ednaj\u00facu hne\u010f 7 dn\u00ed po o\u0161etren\u00ed zrieden\u00fdm v\u010del\u00edm jedom.<\/p>\n\n\n\n<p>Pozorovalo sa, \u017ee tento \u00fa\u010dinok bol ocenen\u00fd intratek\u00e1lnou injekciou idazox\u00e1nu. Pod\u013ea t\u00fdchto zisten\u00ed mo\u017eno poveda\u0165, \u017ee jednorazov\u00e9 alebo opakovan\u00e9 o\u0161etrenie zrieden\u00fdm v\u010del\u00edm jedom m\u00f4\u017ee zmierni\u0165 chladov\u00fa alod\u00fdniu vyvolan\u00fa CCI aktiv\u00e1ciou spin\u00e1lneho \u03b12-adrenoceptora.<\/p>\n\n\n\n<p><strong>Kombinovan\u00e9 \u00fa\u010dinky akupunkt\u00fary v\u010delieho jedu a morf\u00ednu na neuropatick\u00fa boles\u0165 u my\u0161\u00ed vyvolan\u00fa oxaliplatinou<br><\/strong>BVA (akupunkt\u00fara v\u010delieho jedu) sa \u0161iroko pou\u017e\u00edva na odstr\u00e1nenie bolesti a jej mechanizmus je sprostredkovan\u00fd spin\u00e1lnymi serotonergn\u00fdmi a noradrenergn\u00fdmi receptormi. Mus\u00edte si by\u0165 dobre vedom\u00ed morf\u00ednu \u2013 sl\u00e1vneho opioidu, ktor\u00fd sa pou\u017e\u00edva na kontrolu bolesti. Zatia\u013e \u010do oxaliplatina je chemoterapeutikum, liek ur\u010den\u00fd na lie\u010dbu kolorekt\u00e1lneho karcin\u00f3mu, ale vyvol\u00e1va chronick\u00fa perif\u00e9rnu neuropatiu. V tejto klinickej \u0161t\u00fadii sa sk\u00famalo, \u010di lie\u010dba uskuto\u010dnen\u00e1 v kombin\u00e1cii s vy\u0161\u0161ie uveden\u00fdmi l\u00e1tkami m\u00e1 alebo nem\u00e1 nejak\u00fd druh adit\u00edvneho \u00fa\u010dinku na my\u0161i indukovan\u00e9 oxaliplatinou.<\/p>\n\n\n\n<p>Z d\u00f4vodov hodnotenia sa vykonali mechanick\u00e9 a studen\u00e9 alod\u00fdnie, testy von Freyovho vl\u00e1kna a acet\u00f3n. Kombin\u00e1cia akupunkt\u00fary v\u010delieho jedu a morf\u00ednu v n\u00edzkej d\u00e1vke preuk\u00e1zala dlhotrvaj\u00faci \u00fa\u010dinok ako samotn\u00e1 akupunkt\u00fara morf\u00ednu alebo v\u010delieho jedu vo vysokej d\u00e1vke.<\/p>\n\n\n\n<p>Intratek\u00e1lna predbe\u017en\u00e1 lie\u010dba s nalox\u00f3nom 20 ug (opioidergn\u00fd) alebo antagonistom receptora MDL-72222 15 ug (5 HT3) nakoniec zabr\u00e1nila tomuto adit\u00edvnemu \u00fa\u010dinku. Pod\u013ea testu von Freyho vl\u00e1kna bolo zaznamenan\u00e9, \u017ee morf\u00edn spolu s akupunkt\u00farou v\u010delieho jedu a NS ved\u00fa k p\u00f4sobiv\u00fdm \u00fa\u010dinkom, ktor\u00e9 trvali 120 min\u00fat po lie\u010debnej terapii.
